Dosage Considerations:

Patients with CRPC (castration-resistant prostate cancer) or mCSPC (metastatic castration-sensitive prostate cancer) receiving Xtandi should also receive a gonadotropin-releasing hormone (GnRH) analog concurrently or should have had bilateral orchiectomy.

Patients with nmCSPC (non-metastatic castration-resistant prostate cancer) with high-risk BCR (biochemical recurrence) may be treated with Xtandi with or without a GnRH analog13. Treatment can be suspended if PSA is undetectable (< 0.2 ng/mL) after 36 weeks of therapy and reinitiated when PSA has increased to ≥ 2.0 ng/mL for patients who had prior radical prostatectomy or ≥ 5.0 ng/mL for patients who had prior primary radiation therapy.

If a patient experiences a ≥ Grade 3 toxicity or an intolerable adverse reaction, dosing should be withheld for one week or until symptoms improve to ≤ Grade 2. Then, resume at the same or a reduced dose (120 mg or 80 mg) if warranted.

The concomitant use of strong CYP2C8 inhibitors should be avoided if possible. If patients must be co-administered a strong CYP2C8 inhibitor, reduce the Xtandi dose to 80 mg once daily2. If co-administration of the strong inhibitor is discontinued, the Xtandi dose should be returned to 160 mg once daily.
